As a Physical Therapist, you will evaluate, develop, and implement individualized treatment plans for residents and patients with temporary or long-term physical conditions. You will play a key role in restoring function, promoting healing, and improving overall quality of life.
Evaluate patients and develop individualized treatment plans
Plan and administer therapeutic interventions to relieve pain and improve mobility
Restore or enhance functional movement and independence
Ensure all treatments follow the established plan of care
Monitor and document patient progress and response to treatment
Educate patients, families, and caregivers on therapy plans and goals
Maintain accurate documentation in compliance with state and federal regulations
Current Physical Therapist (PT) license (state-specific)
Graduate of an accredited Physical Therapy program
Strong clinical assessment and documentation skills
Excellent communication and teamwork abilities
Experience in skilled nursing or long-term care preferred (but not required)
