Join our dynamic healthcare team as a Home Health Registered Nurse (RN) on a per diem basis, delivering compassionate, expert nursing care directly in patients' homes. Your energetic approach and commitment to excellence will help us uphold the highest standards of home-based healthcare.
Responsibilities
- Admission to DC - case management to ensure seamless transitions of care within the home setting.
- Full patient assessment, including vital signs, physical exam, medication reconciliation
- Direct care - medications via IV infusion, injections, wound care, catheterization (including Foley and urinary catheters), ostomy care, and tube feeding management.
- Manage complex cases involving chronic disease management such as heart failure, diabetes, dialysis care, and infectious disease treatment.
- Educate patients and their families on medication administration, disease management strategies, nutrition plans, and safety precautions tailored to individual needs such as memory care or Alzheimer’s/dementia support.
- Document in WellSky (Kinnser)
Required
- CA RN License 1 + years
- Current Health Screening (6 months)
- TB test (6 months)
- BLS certification
- Proof of Employment
