2026 Position Description
Works with a multidisciplinary team at Harvest House to assist clients experiencing a housing crisis, and/or are at risk of homelessness, providing administrative support for patient/client centered, trauma-informed, holistic support services that assist individuals and families maintain or quickly regain stability in permanent housing after experiencing a housing crisis and/or homelessness.
Responsibilities:
· Comply with HIPAA privacy laws, and NY State/Federal Department of Health and Housing and Urban
Development laws and regulations.
· Assist Directors of Housing and Chief Financial Officer to fulfill grant management and reporting
requirements for the HOPWA and COC Housing programs.
· Assist clients in the ESG-HP program who are at risk of homelessness.
· Assist Directors of Housing to provide housing relocation and stabilization services, short-term and
medium-term rental assistance, and case management to clients based on City of Buffalo and
HUD requirements.
· Develop and maintain a case management filing system and complete data entry for the program
including monthly billing and inputting program statistics in the Homeless Management Information
System (HMIS).
· Connect clients to housing, job training, workforce development, access to healthcare, and other
support services internally at Harvest House and in the community.
· Navigate clients to the Department of Social Services for extra support and/or assistance applying for
benefits.
· Assist Housing Directors to review client case records, providing prompt responses to clients
experiencing challenging personal and health situations.
· Work with a multidisciplinary social work team to provide outreach for safety-net and social service
programs.
· Operate consistently and ethically within the mission, vision and core values of Harvest House.
· Comply with all Harvest House policies and procedures.
· Attend and participate in organizational staff meetings and training.
· Other tasks as assigned by Supervisor.
Qualifications Preferred
- Demonstrated skills in Case Management and Social Services to support individuals and families with housing-related needs.
- Knowledge of Social Housing and Affordable Housing programs, including eligibility requirements and local resources.
- Experience or familiarity with Property Management practices, including lease agreements, tenant rights, and landlord communication.
- Experience with HMIS program
- Strong interpersonal, communication, and conflict-resolution skills, with the ability to build trust across diverse communities.
- Ability to work collaboratively with community partners and multidisciplinary teams while maintaining professional boundaries.
- Organizational and documentation skills, including proficiency with basic computer applications and data tracking tools(ie. Excel, Microsoft Word).
- Commitment to social justice, trauma-informed care, and culturally responsive practices.
- Relevant education or experience in social work, human services, community development, or a related field is preferred.
$22.00/hour
37.5 paid hours per week (must include an additional 30 minutes NYSDOL required unpaid break per shift of more than 6 hours).
Position allows for variable/flexible hours to meet the needs of HOPWA/CoC Clients with the ability to flex hours to attend outreach events during evenings and weekends as required. Not to exceed 40 paid hours per week without prior authorization of direct supervisor.
Paid Time Off – 60 hours
Paid Holidays (7.5 hours each): New Year’s Day, MLK Day, President’s Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Day
Paid Floating Holidays: 15 hours (7.5 hours each)
Paid Sick Time: 40 hours
Medical Insurance: Harvest House will contribute up to $5,000 annually toward a company sponsored health plan
