Licensed Vocational Nurse (LVN) – Skilled Nursing Facility

Position Summary
We are seeking a compassionate, dependable, and motivated Licensed Vocational Nurse (LVN) to join our Skilled Nursing Facility team. The LVN is responsible for providing high-quality nursing care to residents, administering medications and treatments, monitoring changes in condition, and supervising CNAs during the assigned shift.
We are looking for a team player who is committed to resident-centered care and can work effectively in a fast-paced skilled nursing environment.
Responsibilities
- Provide direct nursing care in accordance with resident care plans and physician orders.
- Administer medications and treatments accurately and timely.
- Monitor residents for changes in condition and promptly notify the physician, RN, and/or appropriate provider.
- Complete accurate and timely nursing documentation.
- Perform assessments and routine nursing observations within LVN scope of practice.
- Supervise and provide direction to CNAs during the assigned shift.
- Ensure resident care is completed appropriately and timely.
- Communicate effectively with residents, families, physicians, pharmacy, therapy, and other members of the interdisciplinary team.
- Receive, transcribe, and implement physician/provider orders.
- Assist with admissions, transfers, and discharges.
- Follow infection prevention and Enhanced Barrier Precaution requirements.
- Respond appropriately to emergencies and changes in resident condition.
- Maintain medication and treatment carts in accordance with facility policy.
- Complete incident reports and required follow-up documentation.
- Participate in resident care conferences and interdisciplinary communication as needed.
- Maintain resident dignity, privacy, safety, and rights at all times.
- Follow all facility policies and applicable federal and California regulations.
- Perform other nursing duties as assigned by the Director of Nursing or designee.
Qualifications
- Current and active California LVN license – Required
- Current CPR/BLS certification
- Skilled Nursing Facility or Long-Term Care experience preferred
- PCC/PointClickCare experience preferred
- Strong clinical assessment and documentation skills
- Excellent communication and organizational skills
- Ability to supervise CNAs and work collaboratively with the nursing team
- Ability to manage multiple priorities in a fast-paced environment
