Job Summary
Pay would be 70 per visit, plus mileage reimbursement. We are seeking a dedicated and energetic Mobile Physical Therapist to join our dynamic healthcare team. In this role, you will provide comprehensive physical therapy services directly in patients' homes and community settings, promoting recovery, mobility, and independence. Duties
- Assess patients’ physical conditions through detailed evaluations, including mobility, strength, and functional abilities
- Develop and implement individualized treatment plans tailored to each patient’s needs, goals, and medical history
- Administer therapeutic interventions such as manual therapy, trigger point therapy, and exercise programs to promote healing and restore function
- Conduct discharge planning to ensure seamless transition from active treatment to ongoing self-care or community resources
- Collaborate closely with physicians, nurses, occupational therapists, and other healthcare providers to optimize patient care strategies
- Maintain accurate clinical documentation adhering to standards for medical terminology, physiotherapy records, and electronic health records (EHR) management
- Educate patients and caregivers on exercises, safety measures, and home modifications to support recovery and prevent future injuries
Experience
- Proven experience practicing as a licensed physical therapist in outpatient clinics, inpatient hospitals, or home health settings
- Demonstrated expertise working with diverse patient populations including geriatrics, pediatrics, neurology cases, orthopedics, and post-surgical rehabilitation
- Strong background in acute care environments with familiarity in discharge planning and hospital-to-home transitions
- Knowledge of physiology, anatomy, medical terminology, and clinical documentation standards necessary for accurate record keeping
- Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for efficient documentation and communication
- Skilled in developing treatment plans that incorporate physiotherapy techniques such as trigger point therapy and patient rehabilitation strategies
- Ability to effectively communicate with clients of all ages while providing compassionate home & community care
