We are seeking a compassionate and highly skilled Hospice Registered Nurse Case Manager to join our dedicated team. Responsibilities
- Conduct thorough patient assessments, including vital signs, physiological evaluations, and symptom management to develop personalized care plans.
- Coordinate interdisciplinary team efforts to deliver holistic hospice care that aligns with patient wishes and medical needs.
- Manage patient intake and discharge planning, ensuring smooth transitions between inpatient, outpatient, or post-acute settings.
- Monitor and document patient progress accurately in medical records while adhering to HIPAA regulations to protect patient confidentiality.
- Collaborate with primary care physicians, specialists, and community resources to optimize patient comfort and address complex medical issues such as infectious diseases or behavioral health concerns.
- Oversee utilization management and diagnostic evaluations to ensure appropriate resource utilization and effective symptom control.
- Provide education and support to patients and families regarding disease progression, medication management, and advance directives.
Qualifications
- Current RN licensure with active registration in the state of CO.
- Critical care experience preferred, including ICU or emergency room background; experience in Level I or Level II trauma centers is highly valued.
- Strong knowledge of physiology, medical terminology, and acute care nursing practices across hospital medicine, medical-surgical units, PICU (Pediatric Intensive Care Unit), or behavioral health settings.
- Proven experience in case management, discharge planning, post-acute care coordination, or outpatient clinic environments.
- Familiarity with managed care processes, utilization management protocols, diagnostic evaluation procedures, and medical records documentation.
- Excellent patient assessment skills combined with the ability to handle complex cases involving geriatrics, pediatrics, or infectious disease care.
- Knowledge of HIPAA compliance standards and proficiency in medical terminology is essential.
- Ability to work independently while collaborating effectively within multidisciplinary teams; strong communication skills are a must.

Pay: $85,000.00 - $100,000.00 per year
