Homeside Rehab is seeking compassionate and motivated Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) to provide one-on-one home-based therapy services in Sundown, NY 12740.

Provide skilled Physical Therapy services in the patient's home
Conduct evaluations and develop individualized plans of care, as applicable to your professional scope of practice
Carry out established treatment plans and therapeutic interventions
Provide therapeutic exercises, strengthening, balance training, gait training, mobility activities, and other interventions within your professional scope of practice
Monitor patient progress and communicate changes or concerns to the appropriate supervising therapist and clinical team
Educate patients and caregivers on safety, mobility, home exercise programs, and functional independence
Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements
Communicate professionally with patients, caregivers, therapists, and office staff
Provide one-on-one therapy services in the patient's home
Active New York State Physical Therapist (PT) or Physical Therapist Assistant (PTA) license, as applicable
Basic computer proficiency for electronic documentation
Strong interpersonal and communication skills
Ability to work independently while following established plans of care and supervision requirements
Valid driver's license and reliable transportation for home visits
Home health, outpatient, rehabilitation, or skilled nursing experience preferred
Minimum of 1 year of clinical experience preferred
Job Type: Full-Time, Part-Time, Per Diem
Location: Sundown, NY 12740
Pay: $45–$100 per hour, based on discipline, experience, and case assignment
Schedule: Flexible – choose your availability
Shifts: Day and evening availability, Monday through Friday, with optional weekend availability
