HospiceCare is seeking a compassionate and dedicated Social Worker to provide comprehensive social services and advocacy for patients and their families. This role serves as a liaison within the social service network across the counties served by HospiceCare, helping patients navigate resources while providing emotional support during their hospice journey.
Position Summary
The Social Worker provides comprehensive social service and advocacy services for assigned patients and families as needed. This position collaborates with community agencies, healthcare professionals, and the interdisciplinary care team to ensure patients and families receive the support and resources they need.
Qualifications
- Graduate of an accredited school of social work
- Bachelor of Social Work (BSW) required; Master of Social Work (MSW) preferred
- Current unrestricted West Virginia Social Work license
- Grief counseling experience preferred
- Home health and/or hospice experience preferred
- Employment is contingent upon successful completion of a drug screen, background check, and other applicable pre-employment screenings
Work Environment
- Office and healthcare facility settings
- Frequent interaction with patients, families, staff, and community resources
- Exposure to emotionally sensitive and stressful situations
- Regular use of computers, phones, and other office equipment
Physical Requirements
- Ability to sit, stand, and walk throughout the workday
- Ability to communicate effectively with patients, families, and team members
- Ability to occasionally lift and carry materials weighing up to 20 pounds
