Provides resident care in accordance with standards of practice and within the scope of LPN license.

Observes and reports on residents' conditions/changes and documents in accordance with facility policy and as required by DHH regulations.

Administers medication and treatment as prescribed.

Receives, transcribes, and executes physician orders and follows all 3rd party instructions.

Implements and evaluates resident's plan of care upon intake, status change and every ninety (90) days.

Invent01Y and ordering of equipment and supplies; notifies supervisor when supplies are needed.

Ensures appropriate and timely response to resident and/or family concerns.

Works closely with the Director of Nursing and Medical Director as needed.

Informs CNA/PCA staff about the condition of residents and the expectations and needs for the shift.
Required Skills/Abilities
-
Display dependability by being punctual and maintaining consistent attendance.
-
Possess basic computer and electronic ability in order to complete the required onboarding, training and job-specific tasks.

Ability to work closely and efficiently with other CNAs/PCAs and all Francois Bend staff.

Clear communication and teamwork with all departments to provide well-rounded care.
Qualifications
-
Must have a current, valid LPN license.

Education and/or experience in long-term care, acute care or geriatric nursing preferred.

Must possess the ability to follow verbal and written instructions, correspondence, and memos.

Must have reliable transportation.

Must be CPR and First Aid Certified.

Must be able to successfully pass a background check
