MAIN DUTIES AND RESPONSIBILITIES:
- Acts as a liaison between youth, family, program and placing agency representatives.
- In consult with the Treatment Director, communicates with placing agency representative, family, and treatment team members about significant occurrences involving the youth and his family.
- Maintains and documents monthly contact with each family; offers parenting support and facilitates communication and visitation between parents, family, and a youth’s friends.
- Acts as primary liaison between the program and placing agency representatives. Documents all phone and personal contact with youth, family members, legal personnel or program staff in the youth’s record.
- Coordinates and maintains a schedule of service planning meetings, sends notifications to families and placing agency representatives, and documents all contacts in the youth’s record.
- Completing an admission assessment or any other evaluation of a child for placement;
- Developing, reviewing, and updating of service plans for a child in care;
- Completing a discharge or transfer summary for a child;
- Responsible for maintenance of clinical and medical youth charts.
- Data entry.
- On-call at least one week out of a month.
- Report suspected abuse, neglect, and exploitation to the Child Abuse Hotline.
- Know and comply with applicable rules and regulations related to Chapter 748 Minimum Standards, Chapter 42 of the Human Resources Code, Chapter 745 Minimum Standards relating to Licensing, related contract standards and requirements, Gentle Mental’s policies and procedures, and any other laws which are relevant to the employee’s duties and responsibilities.

KEY WORKING RELATIONSHIPS:
May include but not limited to, youth in care, parents or guardians and families of youth in care, representatives from HHSC Licensing, representatives from contractual entities, representatives from DFPS, all employees, contracted providers and community providers.
Experience:
- One year of documented full-time work experience in a treatment setting serving children, including RTCs, child-placing agencies providing treatment services, psychiatric hospitals serving children, etc.
- Must be at least 21 years old, pass all required background clearances, submit the required criminal history affidavit, be physically, mentally, and emotionally capable of performing assigned tasks and have the skills necessary to perform assigned tasks, and demonstrate competency, prudent judgment, and self-control in the presence of youth and when performing assigned responsibilities.
