Your work will not be limited to treating symptoms. You will help patients understand their health, manage chronic conditions, prevent future complications, and receive the coordinated care they need.
What You Will Do
As a Family Medicine Physician at Tri-State, you will provide comprehensive, compassionate, and evidence-based primary care within your professional training and scope of practice.
Your responsibilities will include:
- Provide primary and preventive care to children, adults, and seniors
- Perform comprehensive medical examinations and health assessments
- Diagnose and treat acute illnesses, injuries, and common medical conditions
- Manage chronic conditions such as diabetes, hypertension, asthma, high cholesterol, and heart disease
- Develop individualized treatment and follow-up plans
- Order and interpret laboratory tests, imaging studies, and other diagnostic services
- Prescribe medications and monitor their effectiveness and safety
- Provide preventive screenings, health education, and wellness counseling
- Educate patients about diagnoses, medications, treatment options, and lifestyle changes
- Identify patients who require specialty care and coordinate appropriate referrals
- Follow up on test results, referrals, hospital visits, and changes in patients’ conditions
- Recognize urgent medical concerns and respond appropriately
- Maintain complete, accurate, and timely electronic medical records
- Follow clinical guidelines, patient-safety standards, HIPAA requirements, and applicable healthcare regulations
- Collaborate with clinical staff, medical assistants, administrative team members, and outside specialists
- Participate in quality-improvement, population-health, and patient-safety initiatives
- Support Tri-State’s commitment to improving healthcare access and patient outcomes
- Participate in clinical meetings, training, and professional-development activities as required

Required Qualifications
- Doctor of Medicine or Doctor of Osteopathic Medicine degree from an accredited medical school
- Successful completion of an accredited Family Medicine residency program
- Board-certified or board-eligible in Family Medicine
- Current and unrestricted medical license in the state of the assigned clinic, or eligibility to obtain the required license before beginning employment
- Current DEA registration, or ability to obtain it before starting when required
- Current Basic Life Support certification, or ability to obtain certification before starting
- Ability to complete Tri-State’s credentialing, privileging, and payer-enrollment requirements
- Strong knowledge of primary care, preventive medicine, and chronic-disease management
- Strong clinical judgment, communication, and decision-making skills
- Ability to maintain complete, accurate, and timely medical documentation
- Ability to provide respectful care to patients from diverse backgrounds
- Commitment to ethical, compassionate, and patient-centered medicine
Preferred Qualifications
- Previous experience practicing Family Medicine or outpatient primary care
- Previous FQHC, community-health, public-health, or safety-net experience
- Experience treating patients across different stages of life
- Experience managing medically complex patients and chronic conditions
- Familiarity with Medi-Cal, Medicaid, Medicare, and community-health documentation requirements
- Experience using electronic health-record systems
- Bilingual communication skills are helpful but not required

Work Environment
This is an on-site, outpatient Family Medicine position within a community healthcare setting.
The physician will work closely with clinical and support staff to provide coordinated, patient-centered care. The exact clinic schedule, patient volume, call expectations, and location-specific responsibilities will be discussed before employment.
