Job Summary
We are seeking a dedicated and energetic Lead Care Manager/Housing Navigator to join our team on a full-time or part-time basis. In this vital role, you will serve as a compassionate advocate and expert resource for individuals experiencing homelessness or housing instability. Your leadership will help connect clients with housing assistance programs, social services, and community resources, ensuring they receive comprehensive support tailored to their unique needs. This position offers an exciting opportunity to make a meaningful impact in underserved communities by guiding clients through complex systems and advocating for their housing stability and well-being.
Responsibilities
- Lead case management efforts by developing individualized service plans that address clients’ housing, health, and social needs.
- Act as a primary point of contact for clients, providing ongoing client communication and crisis intervention when necessary.
- Navigate and coordinate access to housing assistance programs such as LIHTC (Low-Income Housing Tax Credit), Section 8, and other HUD (Housing and Urban Development) regulations.
- Collaborate with landlords, property managers, and community partners to negotiate lease agreements while ensuring compliance with Fair Housing laws and landlord-tenant law.
- Conduct assessments related to high-risk populations, including individuals with mental health conditions or substance use challenges, utilizing motivational interviewing and crisis management techniques.
- Maintain detailed records using property management tools like OneSite and document all interactions in accordance with social services standards.
- Provide training or mentorship to team members on culturally competent care practices and effective client engagement strategies.
Skills
- Strong negotiation skills to facilitate lease agreements and resolve conflicts effectively.
- Expertise in crisis intervention, crisis management, and addiction counseling techniques.
- Knowledge of landlord-tenant law, Fair Housing regulations, and HUD policies related to housing assistance programs.
- Proficiency with Microsoft Office Suite (Word, Excel, Outlook) and property management software such as OneSite.
- Experience working with culturally diverse communities and underserved populations experiencing homelessness or housing insecurity.
- Ability to support high-risk populations through case management strategies tailored to mental health conditions or substance use issues.
- Excellent client communication skills combined with motivational interviewing techniques to foster trust and engagement.
Qualifications
- Bachelor’s degree in Social Work, Psychology, Nursing, or a related field (Master’s preferred).
- Minimum of 2 years of experience in case management, social services, or a related field.
- In-depth knowledge of local health and social service resources in Contra Costa County.
- Strong proficiency in care coordination principles, electronic health records, and case management software.
- Excellent communication, interpersonal, and organizational skills; bilingual abilities are a plus.
- Valid driver’s license and reliable transportation.
