Community Health Network has been placing experienced nurses in home care, hospice, and case management for over 30 years. We partner with the top home care & hospice agencies, health plans and health systems in MA.
We are currently assisting a top health plan out of the greater Boston area with a need for an RN Care Manager for a Senior Care Options (SCO) plan. The RN Care Manager will do Assessments and Care Manage their members remotely (telephonically) as well as do in field member Assessments that would be in the designated territory.
Requires current or recent RN experience as a Home Care Nurse Case Manager and/or a RN Case Manager with a Senior Care Options plan.
- Monday-Friday, 40 hours/week
- Training and all equipment necessary is provided for this position.

Remote based but will require field visits to members homes or faciltiies on the North Shore in the Beverly/Danvers/Peabody/Wenham/Newburyport MA and surrounding areas to conduct assessments when as needed. (This is not a fully remote position)
Must reside and/or be commutable to the North Shore MA areas.
Minimum 3 years Home Care RN Case Manager experience with a Medicare Certified agency and/or current or recent SCO plan experience.
