Position Overview
We are seeking a Licensed Professional Clinical Counselor (LPCC) or Licensed Professional Social Worker (LPSW) to join our growing behavioral health team. In this role, you will provide individual therapy, psychosocial assessments, and supportive counseling to residents navigating complex mental health, adjustment, and end-of-life challenges. This position offers autonomy in clinical practice, a collaborative team environment, and the opportunity to make a meaningful impact in the lives of older adults.

Responsibilities
- Conduct comprehensive psychosocial assessments and develop individualized treatment plans.
- Provide individual and group therapy addressing depression, anxiety, grief, adjustment disorders, and behavioral disturbances.
- Support residents and families through transitions, crisis intervention, and end-of-life planning.
- Collaborate with Aeris providers, facility staff, and interdisciplinary care teams to ensure integrated, whole-person care.
- Participate in care plan meetings and contribute behavioral health expertise to team-based care.
- Maintain accurate clinical documentation in compliance with state, federal, and facility requirements.
- Support quality improvement initiatives and patient satisfaction goals.
Qualifications
- Current LPCC or LPSW license in Minnesota.
- Experience in a skilled nursing facility, long-term care, or geriatric setting preferred.
- Strong background in geriatric mental health, grief counseling, and behavioral intervention.
- Familiarity with MDS, care planning, and LTC regulatory requirements a plus.
- Excellent communication, collaboration, and problem-solving skills.
