In this vital role, you will provide compassionate, hands-on care to individuals across various settings, including assisted living communities, nursing homes, and home health environments. Your expertise will support residents with daily activities, monitor their health status, and ensure their comfort and safety. Responsibilities
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance
- Observe and document patient conditions accurately using Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems
- Monitor vital signs such as blood pressure, pulse, temperature, and respiration regularly to track health status
- Support medication administration following proper protocols and medical documentation procedures
- Provide specialized care for individuals with dementia or Alzheimer’s disease, including memory care techniques and behavioral management
- Perform catheter care, wound care, and other medical tasks such as Hoyer lift operation or hospice support as needed
- Assist with meal preparation, feeding, cleaning, laundry, and maintaining a clean and safe environment for residents
- Follow HIPAA guidelines strictly to protect patient confidentiality at all times
- Collaborate with nursing staff to ensure comprehensive patient care plans are implemented effectively
- Engage in patient observation to identify changes in condition or behavior that require escalation
Experience
- Prior experience working in assisted living facilities or nursing homes is highly preferred
- Demonstrated knowledge of medical terminology and basic medical procedures
- Experience with hospice care, dementia or Alzheimer’s care, and memory support is advantageous
- Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
- Hands-on caregiving experience involving ADLs, medication administration, vital signs monitoring, and patient observation
- Working knowledge of long-term care practices and home & community care environments
- Certifications such as CPR (Cardiopulmonary Resuscitation), first aid training, or CNA licensure are required or highly valued
- Ability to work with individuals with disabilities or developmental challenges compassionately and professionally
