Licensed Practical Nurse (LPN)

Responsibilities:
Provide direct patient care under the supervision of a registered nurse, including administering medications, wound care, and other treatments as prescribed
Monitor and report changes in residents' conditions, including vital signs, physical abilities, and emotional well-being
Develop and implement personalized care plans to meet the unique needs of each resident
Collaborate with interdisciplinary teams to ensure accurate and effective communication and care coordination
Assist with activities of daily living, such as bathing, dressing, and grooming, as needed
Maintain accurate and up-to-date records of patient care and treatment
