Duties
- Conduct thorough patient assessments to evaluate emotional, social, and psychological needs within hospice settings
- Provide individual, family, and group counseling utilizing clinical techniques such as motivational interviewing, cognitive-behavioral therapy, and crisis intervention
- Coordinate with interdisciplinary teams to develop and implement personalized care plans aligned with hospice and palliative medicine standards
- Assist patients and families with discharge planning, advance directives, and navigating community resources such as assisted living facilities or home care services
- Maintain accurate medical documentation in compliance with HIPAA regulations and medical records standards
- Facilitate communication between patients, families, healthcare providers, and community organizations to ensure seamless care delivery
- Support staff training on social work practice standards, cultural competency, and ethical guidelines related to end-of-life care
Qualifications Master’s degree in Social Work (MSW) from an accredited program; licensure or certification as required by state law for medical social workers
- Proven experience working with geriatric patients, hospice care, or in palliative medicine environments
- Strong knowledge of medical terminology, ICD-9 coding (or current coding systems), Medicare regulations, and clinical documentation practices
- Familiarity with behavioral health interventions including behavioral therapy, mental health counseling, and crisis management techniques
- Experience working with diverse cultural backgrounds and individuals with disabilities or developmental disabilities such as autism or dementia
- Proficiency in patient assessment tools, case management software, and medical records systems
- Ability to work effectively in inpatient settings such as nursing homes or hospitals as well as outpatient clinics or home-based care environments
- Demonstrated skills in client communication, clinical interprofessional collaboration, and community resource coordination
- Certifications in basic life support (BLS) and CPR; experience with assisted living facilities or post-acute care services is a plus

Work Location: On the road
