Growing Home Care company located in Yonkers, NY is looking to hire a full-time Care Coordinator for this fast-paced department. Qualified candidates will be energetic, patient and attentive to the needs of our clients.
Job Description:
* Answer incoming calls in a friendly, professional and knowledgeable manner.
Create and maintain client and caregiver schedules. Problem-solving skills needed for creating compatible matches and the development of positive relationships.
- Monitor and log all client and caregiver activity utilizing the software system.
- Follow up with all client and caregiver issues to ensure their problems are resolved in a timely manner.
- Recognize and capture opportunities to increase service hours to enhance and/or increase quality care..
- Maintain open and effective communication with coworkers, caregivers, clients and family members.
- Misc. case work as needed
- Ensure accuracy of data/information in system and files
- As assigned by manager
