Responsibilities
- Conduct thorough patient assessments utilizing clinical judgment, physiology knowledge, and medical terminology to determine care needs.
- Manage patients as a nurse case manager across diverse settings including hospital medicine, outpatient clinics, and post-acute care environments.
- Coordinate multidisciplinary care plans that incorporate diagnostic evaluation, discharge planning, and utilization management strategies.
- Collaborate with healthcare providers to facilitate timely interventions in emergency medicine, ICU, PICU, Geriatrics, behavioral health, and infectious disease care cases.
- Oversee electronic health records (EHR) management to ensure accurate documentation of medical records, vital signs, and patient progress.
- Support compliance with HIPAA regulations and hospital policies while maintaining patient confidentiality.
- Engage in patient service activities such as intake assessments, managing complex cases involving managed care or hospital experience, and providing personalized patient care.
Experience
- Proven experience in acute care environments such as ICU, emergency room, or Level I/II trauma centers with a focus on trauma or critical care nursing.
- Demonstrated expertise in managing diverse patient populations including pediatrics, geriatrics, and behavioral health patients.
- Strong background in hospital medicine, medical-surgical units, or outpatient clinics with inpatient experience highly preferred.
- Familiarity with health regulation policies related to discharge planning, utilization management, and medical records management.
- Prior experience in managed care settings or post-acute care environments is advantageous.
- Excellent patient assessment skills combined with a comprehensive understanding of physiology and medical terminology.
- Ability to navigate electronic health records (EHR) systems efficiently while ensuring compliance with HIPAA standards.
