AZA Services is a 245D licensed home and community-based services provider in the Twin Cities metro area. We are committed to person-centered care that empowers individuals with disabilities to live fulfilling, self-directed lives in their communities. We are growing and looking for compassionate, dependable Direct Support Professionals to join our team.
Responsibilities
- Provide direct support with activities of daily living (ADLs) including personal hygiene, meal preparation, and mobility
- Assist with homemaking tasks such as light housekeeping, laundry, and grocery shopping
- Implement individualized service plan (ISP) goals and document progress accurately
- Transport and accompany clients to appointments, community activities, and outings
- Promote client independence, dignity, and self-determination in all interactions
- Communicate regularly with care coordinators and supervisors regarding client needs
- Maintain documentation in compliance with 245D licensing and DHS requirements
- Follow all agency policies, safety protocols, and mandatory reporting obligation
Qualifications
- Must be 18 years of age or older
- High school diploma or GED required
- Must pass a DHS background study clearance
- Valid driver's license and reliable transportation preferred
- Prior experience with individuals with disabilities preferred but not required — we train the right people
- Strong communication skills and ability to follow detailed care plans
- Reliable, punctual, and professional
- CPR/First Aid certified or willing to obtain upon hire
