Director of Social Work
Hudson Wide is seeking an experienced and compassionate Director of Social Work to join our team. This leadership role is ideal for a motivated professional who is dedicated to enhancing the quality of life for our residents and supporting families through every stage of care.
Responsibilities:
Conduct psychosocial assessments for new admissions
Develop and implement individualized care plans
Coordinate interdisciplinary care planning and discharge planning
Complete timely documentation, including admissions, quarterly assessments, and MDS
Investigate and document resident concerns and grievances
Serve as a liaison between residents, families, and the care team
Qualifications:
Bachelor’s or Master’s Degree in Social Work or related field
Healthcare experience required; skilled nursing experience preferred
Social Work licensure preferred
Strong communication, organizational, and interpersonal skills
