Licensed Practical Nurse (LPN) – The Haven at Sky Mountain

25+ hours per week (Part-Time with potential for more hours based on facility needs)

Your Role & Responsibilities:
- Lead and support our nursing and care staff with a positive, team-oriented approach each day
- Perform regular rounds to ensure residents are receiving excellent care and that care plans are being followed
- Keep resident care plans current and reflective of their individual needs
- Assess residents, complete admissions, transfers, and discharges
- Promptly communicate significant changes in residents’ conditions to physicians and management
- Work closely with physicians to ensure residents receive the best possible treatments and medications
- Accurately document care, prepare and administer medications, and maintain proper medication records
- Complete necessary charting, reports, and medical forms in a timely and professional manner
- Manage medication supplies, conduct narcotic counts, and follow all safety and regulatory procedures
- Handle physician orders, including faxed orders and stop orders, with careful attention to detail
What We’re Looking For:
- Active Utah LPN license (or ability to obtain one)
- A caring heart and strong attention to detail
- Team player who brings positive energy to work
- Prior long-term care or senior living experience is a plus, but we’re happy to support new graduates or those transitioning into senior care!
