ESSENTIAL FUNCTIONS:
Provides case management to enhance the quality of client management, maximize satisfaction and promote long term sufficiency. Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s barriers to stability.
RESPONSIBILITIES:
- Coordinating and providing care that is safe, timely, effective, efficient, equitable, and client-centred
- Handling case assignments, initial case plans, weekly review case progress and determine successful exit plans
- Helping clients achieve vital documents
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Help clients make informed decisions by acting as their advocate regarding their housing search
- Develop effective working relations and cooperate with community partners throughout the entire case management process
- Arrange abeyances with social services and governmental agencies
- Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
- Record cases information in HMIS for all and any services provided to include:
Intake: Evaluation, Initial case plan, HSP
Weekly Follow up notes entered
Every 30 days follow up note on initial case plan
Discharge planning notes
- Promote quality interventions and outcomes
- Assess and address barriers during housing problem solving conversation (HSP)
- Adhere to professional standards as outlined by protocols, rules and regulations
REQUIREMENTS:
- – High School Diploma or equivalent, Associate’s or Bachelor’s degrees in related fields being preferred.
· Proven working experience in case management or a related job.
· Excellent knowledge of case management principles.
· Excellent knowledge of computer and documentation
· Effective communication skills
· Excellent organizational and time management skills
· Problem solving skills and ability to multi-task
· Compassionate with teamwork skills
