We are seeking compassionate and dedicated Home Health Aides (HHA) for Hourly positions to provide exceptional in-home care for clients with diverse needs. The ideal candidate will have experience in patient observation, assisting with activities of daily living (ADLs), and working within assisted living or long-term care environments. This role involves supporting individuals with various health conditions, including dementia, Alzheimer's, developmental disabilities, and hospice care, ensuring their safety, comfort, and well-being. Candidates should possess excellent caregiving skills, and a commitment to maintaining patient dignity and independence. Responsibilities- ** MUST HAVE ACTIVE HHA LICENSE IN NJ**
- Provide personalized patient care including assistance with ADLs such as bathing, dressing, grooming, and mobility support
- Monitor and observe patients’ health status, recording vital signs and changes in condition
- Assist with medication reminders following prescribed protocols and maintaining accurate documentation
- Support patients with memory care needs, dementia care, Alzheimer's care, and hospice support
- Use assistive devices such as Hoyer lifts safely to transfer patients
- Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment for clients
- Work collaboratively with healthcare teams to ensure comprehensive home & community care services are delivered effectively
Experience
- Prior experience in home health or senior care is highly preferred
- Experience working with individuals with disabilities or developmental challenges is advantageous
- Background in nursing homes or long-term care facilities is beneficial
- Familiarity with medical terminology, hospice care, dementia/dementia care, Alzheimer’s care, and medical documentation systems (EMR/EHR) is desired
- CPR certification required or willing to obtain upon hire
- Knowledge of working with patients requiring catheter care, vital signs monitoring, and use of Hoyer lifts preferred
- Experience in home & community-based caregiving roles demonstrating patience, compassion, and professionalism
