Garden Springs Healthcare is seeking an experienced, highly organized Assistant Director of Centralized Admissions & Intake to support our centralized admissions operations across our portfolio of skilled nursing and specialty care facilities.
This is a fast-paced, high-impact position requiring someone who can think quickly, independently review complex clinical referrals, understand insurance and reimbursement considerations, and help move referrals efficiently from initial review through admission.
The ideal candidate is experienced in skilled nursing admissions, confident making decisions, highly responsive, and able to balance clinical appropriateness, payer requirements, facility capabilities, and customer service.
This is not an entry-level admissions position. Candidates should have significant experience reviewing SNF referrals and be comfortable independently interpreting clinical documentation.
Support the Director of Centralized Admissions & Intake with daily operations and oversight of the centralized intake department.
Review and process referrals for multiple skilled nursing facilities.
Independently review hospital clinical documentation to determine potential appropriateness for admission.
Understand diagnoses, medications, treatments, therapy needs, respiratory requirements, wounds, IV medications, dialysis needs, behaviors, and other clinical factors affecting placement.
Identify higher-acuity or complex referrals requiring additional facility or clinical review.
Maintain a strong understanding of Medicare, Medicaid, Medicare Advantage, managed Medicaid, commercial insurance, authorization requirements, Medicaid Pending, and other payer considerations.
Verify that appropriate insurance and authorization requirements are addressed prior to admission.
Work closely with facility leadership, nursing teams, business development, hospital liaisons, and case management teams to facilitate admissions.
Maintain consistent communication with referral sources throughout the review and admission process.
Assist with prioritizing referrals and ensuring clinical reviews and admission decisions are completed quickly.
Troubleshoot barriers that could delay an admission and proactively work toward solutions.
Assist with bed management and appropriate placement across Garden Springs facilities.
Provide leadership and support to members of the Centralized Admissions & Intake team.
Help maintain department accountability, organization, responsiveness, and follow-through.
Monitor referral pipelines and ensure outstanding referrals are actively followed until a final disposition is reached.
Help identify opportunities to improve referral conversion and overall admission volume.
Provide professional, friendly, and solution-oriented communication to hospitals, patients, families, and internal teams.
Minimum of 5 years of experience in skilled nursing admissions, centralized intake, healthcare admissions, or a closely related role required.
Strong SNF admissions and clinical referral review experience required.
Ability to independently read and interpret clinical documentation without routine assistance.
Strong knowledge of Medicare, Medicaid, managed care, commercial insurance, authorization processes, and payer requirements.
Experience working with hospital case managers, social workers, discharge planners, and healthcare referral platforms.
Strong understanding of skilled nursing levels of care and common clinical considerations affecting admission.
Previous leadership, supervisory, or centralized intake experience strongly preferred.
Experience with medically complex or specialty SNF admissions is a plus.

This is a remote position; however, it is a highly engaged operational role. The Assistant Director must be readily accessible throughout the workday, maintain strong communication with the centralized intake and facility teams, and be comfortable working in a fast-moving environment where referral and admission priorities can change quickly.
Success in this position requires urgency, independence, attention to detail, and the ability to remain organized while managing multiple priorities.
