Job Summary
Evergreen Post Acute is currently seeking a compassionate and experienced Registered Nurse (RN) to join our dedicated team at our nursing home facility. As a RN, you will be responsible for providing direct patient care to residents, ensuring their comfort, safety, and well-being, and working alongside our healthcare team to deliver the highest quality of care. This is an excellent opportunity for a skilled nurse to work in a collaborative and supportive environment with a focus on patient-centered care.
We are currently hiring for a weekend RN as well as PRN RNs.
Responsibilities
- Provide direct nursing care to residents, including administering medications, treatments, and daily care routines as prescribed by physicians.
- Monitor residents' physical and emotional condition and report any changes to the supervising RN or physician.
- Assist with the development and implementation of individualized care plans, ensuring they reflect residents' needs, preferences, and health goals.
- Perform routine assessments, including vital signs, and document changes in residents' conditions.
- Coordinate and communicate with other healthcare professionals, family members, and staff to ensure the continuity of care.
- Assist with resident activities of daily living (ADLs), including bathing, dressing, feeding, and mobility.
- Ensure the safe and accurate administration of medications, including tracking, documenting, and reporting as needed.
- Maintain accurate and up-to-date resident records in compliance with facility policies and state regulations.
- Respond promptly to emergencies, providing first aid and other interventions as necessary.
- Ensure a clean, safe, and respectful environment for residents, promoting their dignity and independence.
Requirements
- Current, unencumbered RN license in the state of Oregon.
- Prior experience in long-term care, skilled nursing, or similar healthcare setting preferred, but not required.
- Proficiency in IV infusion, and aseptic techniques.
- Familiarity with medical documentation standards and procedures.
- Ability to work effectively in a fast-paced environment
- Strong interpersonal skills to communicate effectively with patients, families, and healthcare team members.

Pay: $42.00 - $48.00 per hour
