Position Summary
The Care Coordinator provides case management and wraparound services for participants enrolled in the Recovery Housing Program. Using a person-centered, trauma-informed, and recovery-oriented approach, the Care Coordinator conducts assessments, develops individualized service plans, coordinates referrals, monitors participant progress, and connects participants to services that promote recovery, housing stability, education, employment, healthcare access, and long-term self-sufficiency.
The Care Coordinator works closely with Certified Recovery Peer Specialists, Behavioral Health Technicians, treatment providers, healthcare professionals, and community partners to ensure participants receive coordinated, high-quality care.
Essential Responsibilities
- Conduct participant intake, eligibility screenings, and comprehensive needs assessments.
- Develop, implement, and update individualized service plans using the Wraparound Model.
- Coordinate referrals for behavioral health, healthcare, MAT/MOUD, housing, employment, education, transportation, benefits, and community resources.
- Monitor participant progress and maintain regular contact to support recovery and housing stability.
- Assist participants with Medicaid, SNAP, SSI/SSDI, workforce development, vocational training, and educational opportunities.
- Identify barriers to recovery, provide advocacy, and coordinate crisis intervention services when necessary.
- Collaborate with program staff and community providers to ensure continuity of care.
- Complete assessments, progress notes, service plans, referrals, and required documentation within established timeframes.
- Maintain compliance with SOR requirements, FARR standards, HIPAA, 42 CFR Part 2, and organizational policies.
- Participate in team meetings, supervision, training, and quality improvement activities.
Minimum Qualifications
Education
- Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Criminal Justice, Public Health, or a related field; or
- Associate degree with relevant case management or behavioral health experience.
Experience
- At least one year of experience in case management, behavioral health, recovery support, housing services, or a related human services field.
- Experience working with individuals with substance use disorders and young adults is preferred.
Performance Expectations
- Maintain active participant engagement and timely follow-up.
- Complete required documentation within 24 hours.
- Maintain current service plans for all assigned participants.
- Ensure timely referrals and coordinated service delivery.
- Support participant recovery, housing stability, employment, education, and successful program completion.
Work Environment
This position works in recovery residences, community settings, healthcare facilities, partner agencies, and office environments. Local travel is required to coordinate services and support participant needs.
Pay: $23.00 - $25.00 per hour
