The Heart Failure Hospitalist will provide dedicated inpatient management of patients with advanced heart failure and related cardiovascular conditions. The position is designed to improve access to specialized heart failure care, enhance quality outcomes, support timely consultation services, facilitate transitions of care, and contribute to the continued growth of the Advanced Heart Failure and Transplant Program. The Heart Failure Hospitalist will work collaboratively with cardiologists, advanced heart failure specialists, cardiac surgeons, advanced practice providers, nurses, case managers, pharmacists, and other multidisciplinary team members to deliver comprehensive inpatient heart failure care. Initially, the position will combine inpatient heart failure responsibilities with selected ambulatory clinic sessions. As inpatient volume continues to grow, the role is expected to evolve toward a predominantly hospital-based practice.
Inpatient Clinical Care (approximately 90%)
- Provide inpatient management of patients admitted with acute and chronic heart failure, including
patients on mechanical circulatory support and post-heart transplant.
- Perform and coordinate inpatient heart failure consultations throughout the hospital.
- Assist with management of patients with advanced heart failure, mechanical circulatory support devices, and heart transplantation.
- Participate in multidisciplinary rounds and care coordination activities.
- Facilitate timely evaluation and triage of patients requiring advanced therapies, including LVAD and heart transplantation.
- Collaborate with emergency medicine, hospital medicine, intensive care, cardiac surgery,
electrophysiology, interventional cardiology, and other consulting services.
- Support efficient patient flow, discharge planning, and transitions of care.
- Contribute to the development and implementation of evidence-based heart failure pathways and protocols.
- Participate in quality improvement initiatives focused on heart failure outcomes.
