LVN Nurse Case Manager
Boulder Creek Post Acute | Poway, CA
Monday–Friday | $35–$45 per hour
Boulder Creek Post Acute is seeking an LVN Nurse Case Manager to join our clinical team. This Monday–Friday position plays an important role in coordinating each resident's plan of care, supporting communication across the interdisciplinary team, and helping residents achieve the best possible outcomes throughout their stay.
As the Nurse Case Manager, you'll work closely with residents, families, physicians, nursing, therapy, and other departments to help ensure care is coordinated and goals remain on track. What You'll Do
- Coordinate and oversee resident care plans from admission through discharge
- Collaborate with nursing, therapy, physicians, and ancillary departments to ensure continuity of care
- Communicate with residents and families regarding care plans and discharge planning
- Participate in interdisciplinary care conferences
- Monitor documentation and assist with care coordination to support quality outcomes
- Help facilitate safe and timely resident transitions
- Maintain compliance with facility policies and regulatory requirements
Qualifications
- Current California LVN license in good standing
- Case management, MDS, admissions, discharge planning, or skilled nursing experience preferred
- Strong communication and organizational skills
- Ability to work collaboratively in a fast-paced healthcare environment
Compensation & Schedule
- Monday–Friday schedule
- $35–$45 per hour (LVN)
- Registered Nurses (RNs) are encouraged to apply. RN compensation is negotiable based on experience and qualifications.
