Heights Care & Rehab is seeking an exceptional LPN Unit Manager to lead one of our nursing units and help drive outstanding resident care, team engagement, and clinical outcomes.

Oversee daily operations of an assigned nursing unit
Lead, coach, and support nurses and nursing assistants
Ensure exceptional resident care and customer service
Monitor resident conditions and coordinate care planning
Work closely with physicians, therapists, families, and interdisciplinary team members
Assist with admissions, discharges, care conferences, and resident follow-up
Maintain regulatory compliance and documentation standards
Help improve clinical outcomes, resident satisfaction, and team performance
Foster a positive, accountable, and supportive culture within your unit
Active Colorado LPN License
Strong leadership and team-building abilities
Excellent communication and organizational skills
Resident-focused mindset and commitment to quality care
Ability to prioritize and thrive in a fast-paced environment
Skilled nursing, rehabilitation, or long-term care experience preferred
Previous supervisory or leadership experience is a plus
