We are currently searching for a Patient Care Coordinator to join our team at Personal Home Healthcare and Hospice. The successful candidate will be responsible for assisting with daily office duties of a busy home health and hospice agency.
Hours for the position are Monday through Friday 9am to 5pm, no weekends or holidays.
Responsibilities:
- Answer incoming telephone calls and direct them to the appropriate department
- Complete welcome calls to patients to schedule appointments and verify demographic, contact, insurance, and physician information
- Fax physician orders to providers and complete necessary follow-up in order to ensure timely receipt
- Upload and maintain laboratory results and other clinical documents within the electronic medical records system (EMR)
- Monitor and respond to communications via email, text, and fax
- Assist with data entry of new patient referrals
- Provide administrative support to intake and scheduling departments as needed
Qualifications
- High school diploma or equivalent required
- Proficiency with EMR/EHR systems, Microsoft Office, and email communications
- Previous experience in healthcare administration or medical operations preferred
- High attention to detail and ability to maintain confidentiality of patient information
- Ability to work independently while also working as part of the healthcare team
