Genesis Medical Health is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its growing medical team providing primary care services to geriatric and post-acute patients within a skilled nursing facility in Oneonta, NY .
This is an excellent opportunity for a provider looking for a consistent 3-day-per-week schedule , strong earning potential, and the ability to practice independently while being supported by an established medical team.
3 days per week in the facility
Consistent daytime schedule
Participation in an on-call rotation is required
Approximately 25–30 patients per day

Provide primary, post-acute, and long-term care to skilled nursing facility residents
Perform admissions, follow-up visits, assessments, and chronic disease management
Evaluate changes in condition and address acute medical concerns
Review labs, imaging, medications, and clinical reports
Collaborate with physicians, nursing staff, specialists, families, and facility leadership
Participate in the on-call schedule and remain available for urgent clinical needs during assigned coverage
Help reduce unnecessary hospitalizations through proactive patient management
Complete documentation and EMR charting in a timely manner
Active New York NP or PA license
Ability and willingness to participate in the on-call rotation
Strong clinical judgment and ability to work independently
SNF, nursing home, primary care, internal medicine, or geriatric experience preferred
Comfortable managing medically complex patients
Reliable transportation
