Duties
- Conduct comprehensive assessments of clients’ needs, strengths, and goals to inform personalized case plans.
- Develop and implement action plans that promote client independence and well-being.
- Coordinate with community resources, service providers, and agencies to facilitate access to housing, healthcare, employment, and other support services.
- Monitor client progress regularly, adjusting plans as necessary to ensure positive outcomes.
- Maintain detailed documentation of interactions, service plans, and progress notes in compliance with organizational standards.
- Provide motivational support and guidance to empower clients in overcoming barriers.
- Collaborate with team members and stakeholders to ensure holistic support for each client.
Qualifications
- Proven experience in case management,(minimum of 3 years experience with youth and/or adults) minimum high school diploma or degree in social work, counseling, or related fields.
- Strong interpersonal skills with the ability to build trust and rapport quickly.
- Excellent organizational skills with attention to detail in documentation and follow-up.
- Ability to work independently while managing multiple cases efficiently.
- Knowledge of community resources, social services systems, and relevant regulations.
- Exceptional communication skills—both verbal and written—to clearly convey information and advocate for clients.
- Flexibility to adapt to changing client needs and work environments.
