Position Summary
The Nurse Case Manager is responsible for coordinating, monitoring, and documenting healthcare services for children on a residential campus. This role ensures that all medical care meets regulatory standards and reflects best practices in pediatric and adolescent health.
Key Responsibilities

Coordinate and oversee all medical care for assigned caseload

Ensure compliance with regulatory requirements and internal standards

Obtain and maintain comprehensive medical records, including immunizations and health histories

Communicate with external providers to schedule appointments and gather clinical information

Provide direct nursing care for illness and injury as needed

Facilitate consents for psychotropic medications and non-routine procedures

Serve as a liaison between youth, families, and prescribing providers

Maintain accurate Medication Administration Records (MAR)

Monitor controlled substances, including counts and proper disposal

Support medication distribution and coordinate with pharmacy services

Collaborate with clinical teams and families regarding medical care and treatment plans

Participate in off-site visits (e.g., hospital, specialty appointments) as needed

Provide training to staff, youth, and families on health practices and medication administration
Qualifications

Active New York State RN or LPN license (required)

Experience in pediatric or adolescent care (required)

Medical-surgical, acute, or sub-acute care experience (preferred)

Bachelor’s degree in Nursing or related field (preferred)

Bilingual Spanish strongly preferred
Schedule

Monday–Friday

9:00 AM – 5:00 PM or 8:30 AM – 4:30 PM
Compensation
LPN: $30 – $35 per hour
RN: $75,000 – $92,000 annually
Additional differential for Spanish language skills
