Care Coordinator
- Full-time - Onsite/In-Office Permanent Position
- Monday - Friday, 9a - 5p
- Must have previous experience with Homecare, Health Coach, Case Management/Social Services and or Health Admin/Care coordination
Job Summary:
The Care Coordinator is responsible for supporting the daily operational flow of care delivery by managing the scheduling of supervisory and admissions visits for clinical staff, maintaining clear communication with case management, and serving as the primary liaison between assigned nurses and office staff. This role ensures that visit schedules are organized, confirmed, and executed in a timely manner, while facilitating smooth information exchange across the care team. The Care Coordinator works under the direct supervision of the Care Operations Manager and plays a critical role in maintaining continuity and quality of care.
Qualifications/Educational Requirements:
- High school diploma or equivalent required; associate or bachelor’s degree in healthcare administration, business, or a related field preferred
- 3+ years of experience in healthcare, home care, or administrative coordination role
- Health Coach experience
- Strong verbal and written communication skills
- Proficiency in scheduling systems and basic office software (e.g., AlayaCare, Excel, Microsoft Office, EMR platforms)
- Ability to multi-task, prioritize, and remain organized in a fast-paced environment
- Familiarity with home care operations, nursing staff roles (LPN, RN, PRN RN), and case management processes preferred
Responsibilities/Essential Functions:
The person in this position must be able to perform the following essential job functions with or without reasonable accommodation.
- Schedule and coordinate supervisory visits and admissions visits for LPN, RN, and PRN RN staff in accordance with patient needs and organizational protocols
- Communicate with case management to confirm visit attendance, share scheduling updates, and follow up on any outstanding coordination needs
- Serve as liaison between assigned clinical nurses and office staff, ensuring timely and accurate relay of information between both parties
- Maintain accurate and up-to-date scheduling records in client Overview and spreadsheets for all assigned clinical staff and visits
- Monitor visit confirmations and proactively address scheduling gaps, conflicts, or last-minute changes
- Track and document case management communications and follow up as needed to ensure visits are confirmed and completed
- Support the Care Operations Manager and Clinical Services Manager with scheduling reports, visit tracking, and operational updates as needed
- Assist in coordinating new patient admissions by ensuring all required visits are scheduled and confirmed in a timely manner
- Relay pertinent clinical and operational information between nurses and office staff while maintaining appropriate confidentiality and HIPAA compliance
- Escalate scheduling concerns, attendance issues, or communication breakdowns to the Care Operations Manager promptly
- Participate in operational team meetings and contribute updates related to scheduling and coordination activities
- Perform other duties as assigned by the Care Operations Manager in support of daily care operations
