Position Summary
The Admissions Coordinator is responsible for managing the intake and admission process for patients entering the sub-acute and skilled nursing facility. This role serves as a central point of communication between hospitals, referral sources, patients, families, and internal clinical teams to ensure timely, accurate, and compliant admissions.
Key Responsibilities
- Coordinate all aspects of patient admissions from referral through placement
- Receive, review, and process referrals from hospitals, case managers, and other referral sources
- Verify patient demographics, insurance coverage, and eligibility (Medicare, Medicaid, commercial plans)
- Communicate with hospitals and referral sources to obtain required clinical documentation
- Collaborate with nursing, social work, therapy, and business office teams to determine admission appropriateness
- Open and track authorizations and follow up on pending approvals as needed
- Educate patients and families on admission requirements, facility services, and expectations
- Schedule and coordinate admissions to ensure bed availability and smooth transitions
- Maintain accurate documentation in electronic medical record systems
- Ensure compliance with New York State Department of Health and federal regulations
- Assist with census management and daily admissions reporting
- Support audits, surveys, and internal reviews related to admissions
Qualifications
- High school diploma or equivalent required; Associate’s or Bachelor’s degree preferred
- 1–3 years of experience in admissions, intake, or healthcare coordination
- Experience in a skilled nursing or sub-acute setting strongly preferred
- Working knowledge of Medicare, Medicaid, and managed care plans
- Familiarity with authorization and insurance verification processes
- Strong communication, organizational, and multitasking skills
- Ability to work in a fast-paced, high-volume environment
- Proficiency with EMR systems and Microsoft Office
- Bilingual Spanish required
