We are seeking a Home Health & Hospice Registered Nurse (RN) Case Manager to join our team! We are looking for a dedicated and compassionate Registered Nurse Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate comprehensive patient care, facilitate smooth transitions across various healthcare settings, and ensure optimal health outcomes. Your expertise in clinical assessment, case management, and patient advocacy will drive personalized care plans that meet each patient's unique needs. Responsibilities
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations to inform individualized care plans.
- Manage a diverse caseload across inpatient, outpatient, post-acute, and home health settings, ensuring seamless continuity of care.
- Collaborate closely with healthcare professionals such as physicians, therapists, social workers, and discharge planners to coordinate services effectively.
- Develop and implement comprehensive discharge planning strategies that promote safe transitions from hospital to home or other care facilities.
- Utilize electronic health records (EHR) systems to document patient information accurately while maintaining strict adherence to HIPAA regulations.
- Oversee utilization management processes by evaluating diagnostic evaluations and managing medical records for compliance and quality assurance.
- Provide patient service through clear communication, education on treatment plans, medication management, and addressing concerns with empathy and professionalism.
Requirements
- Valid nursing license with current registration; experience in critical care environments highly preferred.
- Demonstrated expertise in hospital medicine, emergency medicine, ICU, or trauma center settings; Level I or Level II trauma center experience is advantageous.
- Strong knowledge of medical terminology, physiology, infectious disease care, and health regulation policies.
- Proven ability to manage complex cases involving geriatrics, behavioral health, pediatrics, or post-acute care populations.
- Experience with electronic health records (EHR/EHR management), discharge planning, and medical records documentation.
- Excellent patient assessment skills combined with the ability to communicate effectively with clients and healthcare teams.
- Familiarity with managed care principles, utilization management processes, diagnostic evaluation procedures, and outpatient clinic operations.

Pay: $40.00 - $55.00 per hour
