Join our team of healthcare professionals at Cook Hospital & Care Center! We are seeking an experienced and motivated Registered Nurse to serve as our Infection Prevention/Case Manager. This unique position combines infection prevention, case management, utilization review, and discharge planning responsibilities and offers an opportunity to make a meaningful impact on patient care, quality, and safety throughout our organization.
The Infection Prevention/Case Manager is responsible for supporting infection prevention initiatives, case management, utilization review, and discharge planning. This position promotes quality patient care through infection prevention practices, regulatory compliance, care coordination, and effective transitions of care.
Infection Prevention: Responsible for infection surveillance, prevention and control activities, data collection and analysis, education, regulatory compliance, and implementation of strategies to reduce the risk of healthcare-associated infections. Serves as a resource for identifying infection prevention concerns and implementing corrective actions.
Case Management/Utilization Review: Responsible for hospital case management, utilization review, and discharge planning. Coordinates patient care and services with physicians, nursing staff, therapy, patients, families, and outside agencies to promote appropriate utilization of healthcare resources and safe transitions of care.
Qualifications:

Current Registered Nurse (RN) licensure in the State of Minnesota.

Current Infection Control Certification (CIC), or ability to obtain CIC within three years of hire if novice Infection Prevention Practitioner.

Current certification in the Nursing Home Infection Preventionist Training Course, or completion within 90 days of hire.

Associate degree in a healthcare-related field required; Bachelor’s degree preferred.

Experience in infection prevention, case management, utilization review, or discharge planning preferred.

Strong organizational, communication, critical thinking, and interpersonal skills.
Starting Wage: $45.00 - $55.00 per hour, depending on qualifications and experience.
