We are looking for an energetic, professional Referral Coordinator to join our team to join our Team on a Temporary assignment.
Job Summary:
The Referral Coordinator (RC) is responsible for the processing and coordination of patient referrals and is an avid participant of the patient care team. The RC assists providers and patients when necessary with the referral process for specialty care, diagnostic services, and social series to ensure continuity of care utilizing the various protocols required by health plans, DHS and other contracts.

Essential Duties:
· Maintain ongoing tracking and appropriate documentation on referrals.
· Contact specialty partners and insurance companies to ensure prior approval requirements are met. Present necessary medical information such as history, diagnosis and prognosis.
· Be the navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns regarding their referrals.
· Review details and expectations about the referral with patients.
· Remind patients of their approved specialty appointments via mail or phone.
· Ensure that patient's primary care chart is up to date with information on specialist consult reports. Specialty consults must be requested within a specific time period, as per clinic policy.
· Attend Health Plan and contract referral meetings to stay current on processes.
· Participate in PDSA’s to continuously work on improving patient referral processes.
· Avid participation in Patient Centered Medical Home processes.
· Be flexible, organized, and function under stressful situations.
· Follow and adhere to PCHC’s policies.
· Complies with all mandatory trainings.
Job Qualifications and Skills:
· Must be bilingual in English and Spanish with effective verbal and written communication skills
· One year in a related position with Medical terminology and basic anatomy; MA certificate preferred
· Basic computer skills; Knowledge of Electronic Health Record System preferred
· Must be attentive to detail, cooperative, punctual, dependable, and be receptive to new ideas
· Takes initiative to affect positive outcomes

· Effective verbal and written communication skills
· Teamwork oriented
· Organized and able to manage competing priorities

· A valid CA driver’s license is required and proof of insurance in order to drive to various clinic locations and meetings as required.

Pay: $22.25 - $23.34 per hour
