Little Peoples Promises is seeking a Fee-for-Service Early Intervention Service Coordinator to join our growing team. This is an excellent opportunity for an experienced Early Intervention professional or a strong candidate with the right background who is looking to grow in a meaningful, family-centered role.
As a Service Coordinator, you will be the primary point of contact for families in the New York State Early Intervention Program, helping them understand their rights, access needed services, and navigate the Early Intervention process from referral through service delivery.
We are looking for someone who is organized, compassionate, dependable, and committed to helping children and families get the support they need.
Position Summary
The Early Intervention Service Coordinator is responsible for coordinating services for eligible children and their families, monitoring case progress, supporting IFSP development and implementation, maintaining documentation, and ensuring compliance with NYS Early Intervention requirements.
This is a hybrid position with both remote administrative work and community/family-facing responsibilities.
Responsibilities
- Serve as the designated primary contact for assigned families in the Early Intervention Program
- Help families understand their rights and available services within the NYS Early Intervention Program
- Assist families in planning for and obtaining appropriate services based on their child’s needs
- Coordinate, monitor, and follow up on authorized services to ensure timely delivery
- Participate in the development, review, and evaluation of the Individualized Family Service Plan (IFSP)
- Attend IFSP meetings with families and support finalization of service recommendations
- Maintain timely, accurate, and professional documentation of service coordination activities
- Audit and maintain case records, reports, and written communication related to assigned cases
- Communicate with providers, evaluators, and families to support continuity of care and case progression
- Follow all NYS Early Intervention regulations, regional procedures, and company policies
- Attend trainings, staff meetings, and ongoing in-services as required
QualificationsRequired:
- Bachelor’s degree in Psychology, Sociology, Social Work, Human Services, or a related field
- Strong written and verbal communication skills
- Strong organizational and time management skills
- Ability to manage a multi-family caseload and follow through on deadlines
- Willingness to meet with families in their homes and/or community settings
- Strong commitment to supporting children and families
Preferred:
- Current or prior experience as an Early Intervention Service Coordinator
- Knowledge of New York State Early Intervention regulations and procedures
- Bilingual skills
