What You'll Do
- Serve as the primary RN Case Manager for hospice patients throughout their care journey.
- Perform comprehensive nursing assessments and develop individualized plans of care.
- Coordinate care with physicians and the interdisciplinary hospice team.
- Manage symptoms while promoting comfort, dignity, and quality of life.
- Educate and support patients and families through difficult healthcare decisions.
- Collaborate with hospitals, skilled nursing facilities, assisted living communities, physicians, and discharge planners to ensure smooth transitions into hospice.
- Maintain accurate documentation and deliver exceptional clinical care.
What We're Looking For
- Active Minnesota Registered Nurse (RN) license.
- Experience in hospice, home health, oncology, palliative care, or other advanced clinical settings preferred.
- Strong assessment, communication, and critical thinking skills.
- Compassionate, relationship-focused approach to patient care.
- Ability to work independently while collaborating with an interdisciplinary team.
- Valid driver's license and willingness to travel throughout the Greater Twin Cities Metro Area.
