Job Summary
We are seeking a compassionate and highly organized Program Advisor/Case Manager for a direct hire opportunity supporting adults who need access to services, resources, and guidance that promote independent living. This role is based out of an Anaheim office, with regular field work throughout Riverside County.
The Program Advisor/Case Manager will work directly with consumers, community partners, healthcare professionals, and service providers to assess needs, coordinate services, manage individualized plans, and advocate for consumer choice. This is a meaningful opportunity for someone who enjoys helping individuals live more independently while managing a caseload in a field-based environment.

The ideal candidate will be dependable, self-motivated, and comfortable balancing field visits, documentation, service coordination, and consumer advocacy.
Key Responsibilities
- Maintain a minimum caseload of 45 active consumers while monitoring progress and service effectiveness.
- Conduct functional needs assessments and develop individualized support and spending plans.
- Assist consumers with identifying services, accessing community resources, managing budgets, and making appropriate purchasing decisions.
- Support consumers with hiring, training, supervising, and terminating support staff as needed.
- Collaborate with healthcare providers, community-based organizations, service providers, consumers, and representatives.
- Maintain accurate case documentation, records, appointments, deadlines, and field visit schedules.
Compensation and Benefits
- Direct hire opportunity.
- Pay rate: $24.00 per hour. (Budgeted salary, no room for negotiation + mileage reimbursement)
- Schedule: Full-time, 30 hours per week during training then will transition into 40 hours per week.
- Employment type: Non-exempt.
- Office location: Anaheim, CA.
- Field territory: Riverside County.
