Aim Home Health is seeking an experienced, compassionate a Physical Therapist to provide skilled physical therapy services to patients in their homes. This is a PRN, per-visit paid position. The ideal candidate is self-motivated, organized, and experienced in providing home health care while delivering exceptional patient-centered services.
Areas of coverage is DuPage County including Naperville, Lisle, Glendale Heights, Wheaton, Warrenville, and surrounding communities.
Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans.
- Provide skilled physical therapy services in accordance with physician orders and the Plan of Care.
- Assess functional mobility, strength, balance, gait, pain, and safety.
- Implement therapeutic exercises, gait training, balance training, neuromuscular re-education, and other skilled interventions.
- Educate patients and caregivers on home exercise programs, fall prevention, safety, and use of assistive devices.
- Monitor patient progress and modify treatment plans as appropriate.
- Collaborate with physicians, nurses, and the interdisciplinary team to coordinate patient care.
- Complete OASIS assessments and maintain accurate, timely documentation in the EMR.
- Participate in care conferences and discharge planning as needed.
Qualifications
- Current Illinois Physical Therapist (PT) license in good standing.
- Graduate of an accredited Physical Therapy program.
- Home health experience is required.
- Experience with Medicare regulations and OASIS documentation preferred.
- Excellent clinical assessment, communication, and documentation skills.
- Valid driver's license, reliable transportation, and current auto insurance.
