Job Summary
We are seeking a highly skilled and dedicated Clinical Pharmacist to join our healthcare team. The ideal candidate will possess extensive experience in hospital pharmacy practice, including critical care, emergency medicine, and specialized areas such as oncology and pediatrics. This role requires a thorough understanding of pharmaceutical regulations, medication management, and patient-centered care. The Clinical Pharmacist will be responsible for optimizing medication therapy, ensuring regulatory compliance, and providing expert consultation to healthcare providers to enhance patient outcomes across diverse clinical settings.
Duties
- Review and interpret medication orders to ensure appropriateness, safety, and efficacy for a wide range of patient populations including ICU, PICU, trauma centers, and oncology units.
- Collaborate with medical teams to develop and implement individualized pharmacotherapeutic plans aligned with best practices and current clinical guidelines.
- Dispense medications accurately through aseptic techniques, IV infusion preparation, and compounding while adhering to strict pharmaceutical regulatory standards.
- Monitor patient responses to medications, assess potential drug interactions, and adjust therapy as needed to optimize outcomes.
- Educate patients and healthcare staff on medication use, side effects, pharmacology principles, and safety protocols.
- Maintain comprehensive documentation within Electronic Medical Records (EMR/EHR) systems to ensure accurate tracking of medication therapy and compliance.
- Precept pharmacy residents and students, fostering professional development in clinical pharmacy practice.
- Ensure hospital pharmacy operations comply with pharmaceutical regulations and quality assurance standards across all departments.
Skills
- Demonstrated expertise in hospital pharmacy operations with experience in hospital medicine, critical care (ICU), trauma centers (Level I & II), and emergency room settings.
- Strong knowledge of pharmacology, physiology, anatomy, drug regulation policies, and medication administration procedures.
- Proficiency with Pharmacy Information Systems (PIS) and EMR/EHR platforms for efficient medication management and documentation.
- Extensive clinical pharmacy experience including acute pain management, IV compounding, aseptic technique, oncology pharmacotherapy, and pediatrics.
- Familiarity with managed care principles and pharmaceutical regulatory compliance requirements.
- Excellent patient assessment skills combined with the ability to provide comprehensive drug therapy consultations.
- Skilled in practicing as a pharmacist within high-pressure environments such as Level I Trauma Centers or Emergency Departments.
- Experience in precepting pharmacy residents or students is highly desirable to promote ongoing education initiatives.
