You'll be the clinical authority for our clients' care — overseeing medication programs, performing nursing delegations, developing caregiver competency, and ensuring every client's clinical needs are met with precision and compassion. ABOUT THE ROLE
This is a clinically broad, operationally embedded position — not a standard home care staff nurse role. Under Virginia law (12VAC5-381), the Supervising RN is responsible for oversight of personal care services and must be available during all hours personal care services are provided. Combined with the clinical scope below, this is a senior nursing leadership position compensated accordingly.
WHAT YOU'LL DO
Clinical Care & Medication Management:
- Ensure accurate and complete documentation at all times
- Communicate with physicians to obtain and clarify medication orders
- Educate clients and families on changes to care
Client Assessment & Care Planning:
- Conduct nursing assessments for all clients and develop individualized care plans in collaboration with the Clinical Director
- Perform 90-day RN supervisory reviews and document visits
- Oversee hospice care coordination and maintain communication with outside hospice agencies
Caregiver Training & Supervision:
- Conduct weekly medication administration training and orientation for new hire caregivers
- Perform annual medication training for all Home Matters staff
- Train caregivers on all delegated nursing tasks — ensuring documented competency before assignment
- Be available to provide supervision and clinical direction to caregivers on duty during all service hours
- Maintain current knowledge of Virginia regulations governing RN practice, delegation, and in-home care
Compliance & Documentation:
- Ensure all Virginia state regulations governing in-home care are met at all times (12VAC5-381)
- Maintain current knowledge of relevant regulations and communicate changes to agency leadership
WHAT WE'RE LOOKING FOR
- Current Virginia RN license or multistate compact privilege — required by Virginia 12VAC5-381
- Prior experience in home care, home health, or skilled nursing strongly preferred
- Experience with nursing delegation to unlicensed caregiving staff
- Experience teaching and training caregivers or CNAs
- Experience with the senior population
- Prior management or supervisory experience — coaching and motivating staff
- Strong verbal and written communication skills — you will interface with clients, families, physicians, and field staff daily
- Basic computer skills
- Valid Virginia driver's license and reliable transportation required
- Available during all hours personal care services are provided (required by Virginia law)
- Flexible schedule; PRN and part-time availability considered
- Ability to lift 50 lbs. using safe, ergonomic movements
- Bilingual a stron gplus
COMPENSATION & BENEFITS — HOURLY
- Pay: $45–$60/hour, depending on experience and scope
