We are seeking a Registered Nurse to serve as our Care Manager & Operations Lead. This is a leadership role for an experienced nurse who is equally comfortable coordinating care for complex patients and overseeing the day-to-day clinical and administrative operations of our office. The ideal candidate is organized, proactive, adaptable, and enjoys improving processes while supporting both patients and staff.
Clinical Responsibilities
- Perform health risk assessments, including Medicare Annual Wellness Visits.
- Develop and maintain individualized care plans for high-risk and chronically ill patients.
- Coordinate care across specialists, hospitals, home health agencies, and community resources.
- Conduct patient outreach, education, medication reconciliation, and transitional care management.
- Assist with chronic disease management and population health initiatives.
- Provide vaccinations, therapeutic injections, phlebotomy, IV therapy, and other nursing procedures as needed.
- Perform occasional home visits for homebound or medically complex patients when appropriate.
Operations & Leadership Responsibilities
- Oversee the daily clinical and operational workflow of the practice.
- Supervise, mentor, and support medical assistants and other clinical staff.
- Ensure efficient patient flow while maintaining high standards of quality and patient experience.
- Develop and implement office protocols and workflows to improve efficiency and compliance.
- Monitor clinical quality measures and participate in quality improvement initiatives.
- Assist physicians and leadership with operational planning, staff scheduling, training, and performance management.
- Troubleshoot day-to-day operational challenges and work collaboratively to implement effective solutions.
- Support the implementation of new programs, technology, and practice initiatives.
Qualifications/Expectations:
- RN or LPN License
- EHR expert skills including typing ability
