Job Summary
The Home Health Aide provides personal care and assistance in activities of daily living to Home Care patients according to the Plan of Care developed by the Community Health Nurse and/or Rehabilitation Therapist. The Home Health Aide is an integral member of the Home Care team.
Essential Functions
- JOB SPECIFIC COMPETENCIES: Demonstrates the required job specific technical/professional knowledge, skills and abilities. * 1. Provides personal care, or assistance in personal care, to patients in their homes, incorporating all aspects of personal hygiene such as bathing, shampooing, grooming, skin and foot care. 2. Performs other health-related tasks such as feeding, meal preparation, transfers, ambulation, and assistance with the Home Exercise Program as developed by the rehabilitation therapist. Also performs shopping, laundry and/or light homemaking activities when designated. 3. Conducts patient measurements: weight, temperature, pulse, respirations, when instructed per the Home Health Aide Care Plan. 4. Keeps accurate and timely records of visits and activities performed, and reports changes in patient condition to Community Health Nurse or Nursing Supervisor in a timely manner. 5. Performs routine office tasks when not assigned to patient care.

Promote and adhere to Montefiore Nyack Hospital's guidelines regarding a safe and clean work environment for employees, patients, families and visitors.
