RN MDS Coordinator
Job Summary:
We're seeking an experienced and detail-oriented Registered Nurse MDS (Minimum Data Set) Coordinator to oversee the assessment and care planning.
Key Responsibilities:
Coordinate and oversee the completion of the Resident Assessment Instrument (RAI), including MDS, CAAs, and Care Plans
Collaborate with the Interdisciplinary Team (IDT) to complete timely and accurate
assessments
Maintain and communicate the MDS assessment schedule for all payer sources
Participate in daily PPS huddles, weekly Medicare meetings, and month-end billing compliance reviews
Collect and verify assessment data through chart reviews, bedside assessments, and interviews
Ensure all IDT members complete, sign, and date assessments per federal regulations
Monitor nursing documentation, observe care delivery, and assist with chart audits and recommendations

Qualifications
RN licensed in the state of North Carolina
Minimum 3 years of clinical nursing experience, preferably in long-term care or skilled nursing
Current CPR certification (required)
Knowledge of MDS processes, Medicare guidelines, PDPM, and care plan development
Strong communication, leadership, and organizational skills
Integrity and professionalism to collaborate with residents, families, and healthcare teams
