We are seeking a passionate and dedicated Home Health Physical Therapist to join our dynamic healthcare team. Duties
- Assess patients’ physical condition and develop tailored care plans that address their unique needs and goals
- Provide hands-on physiotherapy interventions including therapeutic exercises, manual therapy, and trigger point therapy to improve mobility and reduce pain
- Coordinate discharge planning with healthcare teams to ensure smooth transitions from inpatient or outpatient settings to home care
- Monitor patient progress regularly, adjusting treatment strategies as necessary to optimize recovery outcomes
- Educate patients and their families on injury prevention, proper body mechanics, and self-care techniques to promote long-term wellness
- Maintain accurate documentation of treatments, progress notes, and care plans within electronic medical record (EMR) systems
- Collaborate with multidisciplinary teams including physicians, nurses, and social workers to deliver comprehensive patient-centered care
Skills
- Strong knowledge of physiology, anatomy, and medical terminology relevant to physiotherapy practice
- Proven experience in home health settings, outpatient clinics, inpatient hospitals or acute care environments
- Expertise in developing and implementing effective care plans for diverse populations including pediatrics and geriatrics
- Familiarity with EMR systems for documentation and patient record management
- Excellent patient care skills with a compassionate approach to working in patients’ homes and communities
- Ability to perform discharge planning efficiently while coordinating with healthcare providers for seamless service delivery
- Knowledge of orthopedics, geriatrics, pediatrics, outpatient therapy techniques, and trigger point therapy is highly desirable

Pay: $90,000.00 - $100,000.00 per year
