Come join our growing team! Genus Patris Hospice is looking for dedicated RNs that can cover Fort Bend, Harris, Waller, Chambers and Montgomery counties. This position requires someone who can speak Mandarin, Cantonese or Vietnamese.
* No On Calls needed (we have oncall nurses)
* We pay Mileage
We communicate and coordinate on Teams and use Kinnser for EHR
Responsibilities:
* Conduct initial assessments of patients to gather medical history and current health concerns.
* Provides constant and ongoing care for terminally ill patients by evaluating patient needs, creating care plans, and bereavement support to patients and their families
* RN regularly assesses and reassesses the nursing needs of the Hospice patient.
* RN provides Hospice nursing services, treatments, and preventive procedures.
* Registered Nurse coordinates the implementation of the plan of care for patients residing in SNF, PCH
* Maintain accurate records in compliance with regulations
* Facilitate communication between patients, families, and healthcare teams
* Reconciliates patient's medications every week.
* Orders New Meds/Refills on patient
* Work under supervision of DON
Requirements :
* RN license (home health/hospice experience a big plus)
* Must pass a criminal background check & MVR check.
* Valid state drivers license. Proof of current automobile insurance.
* Current CPR card;
* Bilingual - English and Mandarin/Cantonese/Vietnamese
* TB testing per agency policy
